So, I just had to drop my 2016 Tacoma TR D off at the dealer before it's 5000 mile service because of multiple issues. I typed out this letter and gave it to the service manager plus a video email to his account of my transmission being stuck in fourth gear. He told me there is a recent update for the software which controls the transmission. Last time I brought the truck in for this problem they said the update they currently had was not for my Vin number. Apparently, they have new software since my last visit to the dealership. Here is the list of problems I am having, I handed this to the service rep that way there is zero chance of my complaint being ignored. They also said they were going to check the transmission fluid, even though I asked them to do this on the last trip in and they failed to do this for me.

1. It has a intermittent delayed shift into drive when the vehicle is cold, sometimes it will take 3 to 5 seconds when switching from reverse to drive before there is an actual gear shift.

2. It frequently gets stuck in fourth gear even when there is zero grade to they road and it is noticeably worse when the truck is driven in cold weather, such as the last week we had in Eugene Oregon.

3. This is the worst problem with the truck and it leaves me feeling vulnerable to a collision. This has happened since the first day I bought the truck but it was hard to duplicate, now it is happening more often. Last week the truck decided to lunge forward as I was stopped behind someone at a red light. It was as if someone had put their foot on the gas pedal and gave it gas while my other foot was on the brake. I had to put my foot all the way to the floor to hold the brakes on hard to avoid hitting the vehicle in front of me. This also happened leaving super taco on West 11th, as I tried to stop for traffic as I was leaving the parking lot, the trucks RPMs shot up to around 1400 RPMs as soon as I came to a complete stop, I almost lost the nose of my truck as it was sticking out into oncoming traffic. It's no different feeling than if somebody reached over with a foot and stepped on the gas pedal while I had my foot on the brake. This also happened as I left my driveway before proceeding on to territorial Highway which has traffic going 55 mph. As the truck sits now, it's not safe for my family to be in.

4. The passenger climate control knob is getting very stiff to rotate, causing it to stick more frequently when it is cold in the vehicle. The driver side is also starting to have intermittent rotation stiffness. It's hard to make the passenger climate control knob stop on a specified heat number due to a very stiff climate control knob.

5. When backing up in reverse the truck surges as if someone is pumping the gas pedal. It makes it difficult at times to back up smoothly to a trailer, or having to back up any length of distance smoothly.
I would love to check the transmission fluid myself but it has no dipstick, I am wondering if the truck has a fluid issue?
 
Tsb will fix most of those issues. I had items 1,2,and5 in your list. Also 2016 TRD Off Road. The symptoms alone should warrant the tranny tsb be completed.

What dealer are you taking it to? I'm also in Eugene. I had the original transmission TSB done and it helped, then I also had the Rev 1 done and it was better still. Solved the problem with hunting for gears. I didn't have the surge or the reverse to drive problem you've had.

Kendall Toyota. I just traded it in for a 4Runner. Now I am stuck with a brand-new snug top canopy with no truck for it. If you or anyone you know needs a great deal on a brand-new truck topper let me know, it's for a long bed 2016-17. I couldn't take anymore frustration especially after having the sunroof leak for the second time.

What is Rev1?
It's just the latest and greatest TSB for the transmission. Mid-late year 2016 toyota came out with the TSB-0077-16 they recalled that late 2016 and came out with the TSB-0077-16 REV1. I guess the original TSB-0077-16 wasn't fixing these like they had hoped.

Thx. Maybe rev1 will fix my 4th gear with cruise on problem. Goes in tomorrow.
 
Rev 1 update completed. Have 48 hours to test results before deciding to trade it in for a 2017 TRDOR 6spd. Manual. Dealership can make it happen with same monthly payment. Only 1 problem.... i have 5g worth of upgrades that i will lose in a trade in situation. Wheels,tires,steps,tonneau cover, seat covers, window tinting, etc. Next 48 hours will determine my decision.

I'm right there with you I've got the same extras put into mine. Let us know how it goes and what you decide to do. I hope it fixes it for you.
I have the Rev1 latest and greatest TSB and mine just on Sunday shifted into 3rd gear at 70mph in cruise control going up and over a mountain and held 4500rpm until it crests the top. Then it went back into 4th and sat at 3300 RPM's on flat ground. It still doesn't like 5th and 6th for some reason. I have video and my MPG is going 16.3-16.2-16.1-16 in a matter of minutes and miles haha crazy.
I've taken videos of it but haven't posted anything. Videos of it in CC at 4500rpms for minutes, videos of it in S mode and it automatically shifted into 4th dropping the RPM's to 3400 then I shifted it into 5th dropping to 2200 and 6th dropped to 1600rpms all at 70mph put it back in D and RPM's shot up to 3200-3400rpm's back to 4th gear. I don't get it. My gas mileage is much better when I use S mode vs letting the transmission decide 4th gear cruising at 70-80 is ok. And 3rd gear at 70mph running 4500rpm's while in CC seems crazy to me. Some of the mountains I was going over where miles long and my truck was screaming 4500 the entire time going over in CC. I could put it in S mode and 4th or 5th and it would run at 3400-2600 which seemed like what it should be doing on its own.
 
I have the 2016 TRD sport double cab 4X4. I already had one TSB update to solve the lag issue from reverse to drive, which it did solve. But I too am experiencing the surge in throttle when reversing. It happens every single time!
I have called Toyota customer service and have taken it in to my dealership. There response was that it's normal. The tech said the throttle will fluctuate when the engine switches from port injectors to direct injectors. Now for this to be normal makes no sense to me because it sure as hell doesn't feel normal!!!
 
Test 1 complete. 50 miles at 73mph with CC on in Drive. Performed well. Never locked into 4th, it downshifted on inclines and immediately went to 5 as cresting hill occured and 6th at start of downhill side. And flat runs between hills was steady in 6th at 1900 rpms at 73mph. I will test again tomorrow. If it performs as test 1? I keep it. If it shows any signs of locking in to 4th tomorrow, I will trade. Cautiously optimistic at this moment.

All is well. But haven't had much highway driving lately. My next road trip should verify early results. Eta? October. Lol
